WAIVER OF CLAIMS AND RELEASE OF LIABILITY:
All persons MUST sign a “Release of Liability” prior to participation in any class or rehearsals at “Dance Avenue”. Parents or guardians must sign on behalf of children under the age of 19.
We take you and/or your child’s safety very seriously and endeavour to provide a safe environment for all lessons. However, by its nature dance is a strenuous activity involving body movement and various physical factors including the participant’s own physical and mental condition. You understand a certain amount of risk accompanies any physical activity. *initial*
I agree TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against Mykhaylo Radko, “Dance Avenue”, and its directors, officers, employees, guides and representatives (all of whom are hereinafter collectively referred to as “the Releasees”); *initial*
Medical Attention
In the event of an emergency, I give permission for “Dance Avenue” staff to seek emergency medical attention for myself or my child. Furthermore, I agree to full financial responsibility for the cost of such treatment. *initial*
Photo Release
The “Dance Avenue” reserves the right to use photographs and videos taken during classes, workshops, performances or other affiliated events for the purposes of instruction, advertising, and promoting Dance Avenue and its programs. Students or parents of students who are minors, who do not wish to comply with this policy, must notify “Dance Avenue” prior to participation in class. *initial*
I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT.
I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS AND ASSIGNS MAY HAVE AGAINST THE RELEASES.
